A case of severe asymptomatic hypercalcaemia in IgG kappa multiple myeloma, focal bone disease and inappropriately normal PTH as a differential diagnostic dilemma

Manova Yuliya , Rabin Neil , Baldeweg Stephanie

We present the case of 63 years old female patient who was diagnosed with IG G kappa multiple myeloma in 2007. Cytogenetics investigations revealed high risk for progression of the disease (cytogenetics: 17 p deletion and t (4/14). The patient decided against active treatment. Over 10 years she was kept under observation with no symptoms. Her basal paraprotein levels slowly progressed from 14 g/l up to 18 g/l. In 2017 she developed mild hypercalcaemia (corrected calcium 2.72 m...

McDonnell Tara , Yuliya Manova , Fedele Stefano , Chung Teng-Teng

We present the case and management of a 76 year old man who developed medication-related osteonecrosis of the jaw (MRONJ) as a result of 12 years of bisphosphonate and denosumab treatment for hypercalcaemia in the setting of metastatic parathyroid carcinoma.Within two years of his original parathyroidectomy, he had metastatic recurrence diagnosed with progressive hypercalcaemia; imaging revealed three pulmonary nodules consistent w...

Khan Sidrah , Manova Yuliya , Bomanji2 Jamshed , Chung Teng-Teng

Background: Radioiodine treatment (RIT) for benign functioning thyroid disease is an effective treatment for patients with hyperthyroidism. The safety, preparation, administration and follow-up of treatment demands time and organisation. The planning of this elective treatment is affected by the patientÂ’s availability which makes adequate and timely treatment of the subsequent hypothyroidism challenging. A quality improvement project was therefore set up to mitigate this....

We present a 32-year-old woman with recurrent hypoglycaemic episodes and a history of juvenile onset SLE. Over months, she experienced severe hypoglycaemic episodes with unconsciousness, occuring mainly in the early morning resulting in several hospital admissions. The patient had to take precautions like eating snacks between meals and before going to bed, and even setting an alarm clock to eat a bowl of porridge at 0200 h. She was of normal weight, did not present with any h...
